HOSPICE
Hospice care is provided by an interdisciplinary team that exercises compassion and respect for the patient’s desires for this chapter in their life journey. The interdisciplinary team consists of the hospice medical director, attending physician , nurse, social worker, chaplain, home health aide, and volunteers.

SENIOR DAY CARE
Adult Day Care Centers are designed to provide care and companionship for older adults who need assistance or supervision during the day. Programs offer relief to family members and caregivers, allowing them to go to work, handle personal business, or just relax while knowing their relative is well cared for and safe. The goals of the programs are to delay or prevent institutionalization by providing alternative care, to enhance self-esteem.

SKILLED NURSING

ASSISTED LIVING COMMUNITIES
Assisted living is a residential option for older adults who need help with some of the activities of daily living—things like cooking meals, getting to the bathroom in the middle of the night, keeping house, and traveling to appointments. An assisted living facility may be a good choice if you need more personal care services than you can get at home , but you don’t need the round-the-clock medical care and supervision of a nursing home.
